Quick Help please :

0. My citizenship was acquired through (check one)

_Birth in the U.S. _Naturalization

Give number of certificate, date and place it was issued.

_Parents

Have you obtained a certificate of citizenship in your name?

_Yes _No

If "Yes," give certificate number, date and place it was issued.

WE checked the Naturalization Box, do we have to check also the "Have you obtained a Certificate of citizenship in your name?" Or is the Naturalization Box enough and we just leave it blank??

I believe the question you are referring to is if you received your naturalization through parents, then you have to say if its in your name or not. Don't think that applies to you if you gain naturalization through yourself.

There are three options here:

1. You were born in the US.

2. You became a citizen through naturalization.

3. You were born outside the US, but one or both parents was a US citizen.

If #1 is true then you check the "Birth in the US" box and stop. You're finished with this question.

If #2 is true then you check the "Naturalization" box, and you provide the number from your certificate of naturalization, as well as when and where it was issued, in the box immediately below.

If #3 is true then you check the "Parents" box. Some people in this situation want or need a document from the US government proving they are a US citizen (for instance, if a CRBA was never filed). Those people would have obtained a certificate of citizenship in their name, and they should provide the certificate number, as well as when and where it was issued, in the box provided.

05/01/2013 - Removal of Conditions - APPROVED!

What Jim said, but I will say that it isn't the best worded question.

I was born in the U.S. and called the USCIS "misinformation line" and asked if they needed my birth certificate # and date/place of issue and the only thing the tele-drone would say is "I can't tell you what to put for any answer."

Turns out I needed none of it, but they got it all for being so "helpful".

He was not born in the Us and became a Citizen through Naturalization, so I think I clicked the right box and filled the Certificate info..

SO i just leave the other one Blank ?!
