Changing Surname on Foreign Passport, Uzbekistan

Hey all,

My wife is from Uzbekistan and has traveled there to visit family as well as change her passport. Her Uzbekistan passport has her maiden name on it and now she wants to change it to her married name. However, the passport office in Uzbekistan is requesting a government form stating that her married last name will be changed to her husbands last name.

Need help here guys cause i can not think of anything that would be official in the US and cover what they are requesting. Any suggestions??

marriage certificate is the general one used for name change, other then that if they are looking for other proof from the government not sure how you would get something different.

marriage certificate is the general one used for name change, other then that if they are looking for other proof from the government not sure how you would get something different.

yeah she has the marriage cert but they specifically want something that states she is changing her last name to my name. Strange, but that is the law in Uzbekistan.

I thought today that I may have her contact a lawyer there to see what might be suggested.
