Properly completing form I-751

The sample I-751 provided under "Example Forms" in this website does not enter any information under boxes like "Care Of" or "If your spouse is deceased"

I-751 instructions clearly indicate: State that an item is not applicable with "N/A", if the answer is none write "NONE"

Is this a big deal? should I make sure that all boxes are completed even with a "N/A" or "None"...has anyone submitted this form as illustrated by the sample cited above?

Yes it's important that you indicate N/A or None when the item is not applicable. Otherwise USCIS may assume you forgot to answer that question. Good luck!
