The I-131 is confusing becuase it is used for many different types of Parole, reentry permits, etc. Although they ask for all of these specifics, they issue a multiple-entry Parole document in most cases (for AOS AP applicants). I would say you are requesting AP in case there is a need to visit your family. We attached a separate explanation with one sentence on it. For the dates, unknown seems fine, if there is an attached sheet. We also USED AP with no problems. Good luck!

AOS for my husband
8/17/10: INTERVIEW DAY (day 123) APPROVED!!

ROC:
5/23/12: Sent out package
2/06/13: APPROVED!
