How much to get I-693 transcribed in Tallahassee, FL?

Hello, peeps.

We're getting ready to send in our I-485 application, but still have to wait for hubby's August pay stub and my daughter and I have to get our Philippine medicals transcribed into the I-693 form for signing and sealing.

There are three civil surgeons here in Tallahassee, FL, and the first one I spoke is charging $150 per person (Impact Clinic on Capital Circle NE)...is this a reasonable fee? Your inputs would be appreciated.

It cost us about 60 bucks if I remember right.

They can charge whatever they feel like, unfortunately - so it's best to shop around if you can.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

We paid $22 here in Hawaii. All the CS has to do is have his secretary fill out the I 693 and sign it. Takes about 10 minutes.

Ugh...another clinic I called wants to charge us $275 for a physical...I tried to explain that because our medical examinations are still valid, we do not need a new physical examination...we just need the information transcribed, signed and sealed by the civil surgeon. Man, what a rip off.

Yeah, that sucks. We went through that in Indiana when my husband was applying for AOS. The receptionist at the first office I called didn't believe it was possible my husband didn't need a new physical. I held her hand through that EXTENDED learning process then waited while she asked the doctor what the charge for that would be. $90, iirc, no doubt made up on the spot. This lady was the only civil surgeon in our town, but we found a different doctor about an hour away who knew immediately what we needed and only charged $20 for it. It was well worth the road trip, both to save us money and to deal with a person who was informed and treated us like normal people.

So, definitely DO call around. $150 is absolutely not reasonable, although obviously you'll have to take it if no one else can do better. I think it's really hit or miss.

So frustrating...but I guess I got my answer.

There are three civil surgeons listed in Tallahassee, FL and this is the information I got:

Dr. Wayne A Sampson, Cross Creek Medical

1381-A Cross Creek Circle, Tallahassee, FL 32301

(850) 877-6393

They are charging $175 for transcription + bloodwork + TB skin test. (Any other vaccines you will need they will charge extra) Even if you have completed the tests within the last 12 months, their base fee is fixed at $175 per person.

Dr. Louis R. Bolen

1706 Riggins Road, Tallahassee, FL 32308

(850) 671-3800

They are charging $150 per person if no physical is required (so this is just for transcription), and up to $240 per person if you require a physical + transcription. According to the fellow I spoke to, they have been doing this for a very long time compared to other places...and unfortunately for me, $150 is the cheapest deal I can get. Ugh.

Dr. Mignon L. Emenike, Impact Health Clinic

1965 Capital Circle, NE, Tallahassee, FL 32308

(850) 656-2006

This clinic is actually closest to where we live, but they are charging $275 for the physical; the contact person I needed to speak to was unavailable, so no word yet on how much for just transcription (or if they offer that at all).

Sigh. Why do they make this so hard?

Even though it would be simpler and easier to have your AOS medical documents done in Tallahassee, there are numerous USCIS doctors not that far from you....Pensacola, Lynn Haven(panama city), Jacksonville etc. A 2 or 3 hour drive can save you a lot. Even look at southern Georgia or Alabama. You not need a doctor in your same state....all that matters is they are USCIS approved. We did our I-693a with a doc in Texas.

Hello from Panama City!

best wishes.

Assuming it was only $60 each to get it done in Jacksonville, my husband would have needed $100 of gas to go there and back...plus whatever number of hourly pay he lost to get off work. Not quite the savings we would be looking for, but I appreciate your suggestion.

Went to the Civil Surgeon at the First Choice Wellness Care Center:

1706 Riggins Road, Tallahassee, FL 32308

(850) 671-3800

The staff was very friendly and David Browne (MSN, ARNP) attended to our paperwork, which will be ready this afternoon. While I am not happy with the fees, I was happy with the service. So for folks like me who chose not to drive out to Jacksonville, they are good people to talk to...they don't take insurance, so they are able to charge lower fees on certain services.

Hi,we live down here sa Tallahassee. When we did my AOS,we went to Dr.Sampson.He charged me $25 only for trancribing my vaccination form to I-693.Yun lang ang binayad ko sa kanya..that time meron ako need na vaccine yung HPV #2 kc yung St.Lukes first shot lng binigay sken..so hndi sila tumatanggap ng health insurance for immigration purposes pero in-advice nya kami ng hubby ko na punta sa Patients First para dun i-take yung 2nd shot ko for HPV para magamit nmin insurance..I paid $20 for the co-pay sa Patients First then binigay na ni Dr. Sampson saken yung sealed envelope ng I-693..so total we spent $45 lang.I explained to him at first na ang gusto ko lang i-transcribed sa form I-693 yung vaccination form ko from St.Luke's.Sinabi ko sa kanya na i don't need to re-do medical again kasi valid for 1 year ang medical ko from Pinas...so ang sinabi nya ay need ko yung 2nd shot ng HPV which I did.Yun lang po.

Btw,san ka po dito sa Tallahassee???
