hi guys

well got married 4th Feb and been preparing all the paperwork since.

Have gotten everything they ask for ready and more, only thing confusing me is the I-693 supplement.

Me and my son came into the US on both a K1 & K2 visa and we both had our medicals done before the embassy interview took place in Belgium, before entering the US.

The doc there gave me my x-ray and copies of DS3025, TB test results and HIV results and said to keep hold of them and send them in with my AOS.

Now I read tons of stuff bout this I-693. Now I understand I dont need another medical as my last one is still valid, but can I get away with only sending DS-3025 (vaccination sheet embassy) for the AOS?

Will they deny my application based on it, or RFE me, or just ask us to bring it to the AOS interview?

I have phoned around to get the supplement, but one doc says itll be between $250-500 which I am totally not willing to pay.

The second doc wants me to have a full medical all over and they start from approx $200 and the last doc says that might shots might have to be retaken and was summing up prices which nearly knocked me flat!

I mean spending $800 to file 2 AOS and an EAD and then that on top, plus posting, passportphotos, copies, etc etc, where does it end?

So, anyone took a chance on sending just their DS3025 and got approved without hassle?

BTW am sending through Chicago, whats the experience there? And my biometrics and interview would be in Omaha, Nebraska.....

Any thoughts will be greatly appreciated!!!!

05/02/08 - Immigration again... removing conditions this time....

05/09/08 - My son received his extension letter

05/13/08 - I receive my NOA1 - here for at least another year!

05/28/08 - getting my biometrics done

09/30/08 - GC APPROVED - CARD PRODUCTION ORDERED!!!!!!!!!

My wife also had a full medical examination before her Embassy interview. She presented documentary evidence that she had had all her immunizations, and the clinic doctor gave her a DS-3025 with all the immunizations checked. On January 6,, I sent in copies of the DS-3025 and the immunization records. Last week, I was RFE'd for an I-693. It cost me $165, and although I have very good medical insurance, I will not be reimbursed..

I interpreted the AOS rules to mean that an I-693 was required, but a nurse advised me incorrectly to wait until it was requested.

Several people on the Board have pointed out that I am fortunate that my own doctor, a Board-cerified Internist, was authorzed to do this exam. That's the other issue besides cost -- finding a Civil Surgeon in whom you have confidence.

12 05 05 .... Successful interview -- visa granted

12 24 05 .... Married!

06 22 09 .... Irina takes the Oath and becomes a US Citizen

I didn't send a civil surgeon's report - just the vaccination worksheet that the Colombian doctor gave her (DS 3025) - since we didn't get an RFE, I didn't take one to the AOS either - We got through without it! This was in Denver. Good Luck

These CS doctors are either totally misinformed or they are taking advantage of the situation.

All they have to do is look at the DS 3025 and any shots you may have taken here and fill the 693A. It takes about 10 minutes. Ours was done by the office staff and taken to the doctor for a signature. There was no charge. Most of the reputable docs charge 10 - 30 bucks.

If the USCIS is going to pick the doctors, they really should open up a complaint system to weed the crooks out of the system.

Ditto....same with us. Our interview was in Chicago. Worse thing that could happen is you get an RFE...then send the supplement and you will be approved. This is what has happened with a number of cases that were sent to CSC and approved without an interview.

Well, we just sent in out DS-3025 with our initial AOS package and we didn't get an RFE. We had done in the medical in Scotland just 2 months before we sent in our AOS package.

But we did get an I-693 supplement (not the full medical done) and took that along to our interview. And when we got there, our officer did specifically ask for our supplement, as well as the DS-3025 original.

So for peace of mind, I would definitely just send in the DS-3025 for now and get that out of the way. They shouldn't RFE you, but if they do, then you can just get the supplement done. It's quick and easy.

I chose not to take the risk of sending only the vaccination worksheet with my AOS package. The I-485 form and USCIS' website made it perfectly clear that although I wouldn't need another medical examination, I would need to include a vaccination supplement. I followed their instructions to the letter.

Even though it cost me a whopping US$330 to get a couple of additional shots and the supplement, having the sealed envelope and a copy in my hands gave me peace of mind. I never got an RFE for anything from start to finish. Thank the good Lord.

We didn't send the vaccination supplement and ended up getting an RFE on the AOS application.

It cost us $50 to have the vaccination supplement completed in NYC, although I've heard it's cheaper in other places.
