Yes, this is a huge requirement. Post this in the Phils regional forum since many there may have similar experiences.

You either need him to sign directly to give permission or you need to have the court system in your country to fully remove his parental rights. As long as a father is alive they are presumed to be interested in their children until you have legal documentation otherwise and you will not be allowed to bring a child here

This will not be over quickly. You will not enjoy this.

See above! Good luck!

You need to understand that in any part of this paperwork chase it all comes down to what you can prove, not what you say.

Proof : His name is on the birth certificate

Words : Hes not in her life

Now pretend you are the CO looking/listening to this ... parental abduction means alot here.

You need to prove that he isnt in her life, or that you have sole custody and what he says has no legal bearing. You need a lawyers advise, and you most definetly need a court order stating that you have sole custody and can take the child out of your country. It really isnt that difficult to get sole custody, but the courts need to make sure that the bio daddy doesnt want anything to do with the child, they need to attempt to find him and ask him. If they cant find him, you will be awarded sole custody. But attempts have to be made, because no matter how long hes been out of the childs life, he still has parental rights.
