Yes well I didn't get my vaccination form thingy when I had my medical, any suggestions on what I should say/do? What will they require if I don't get that?

You can try to contact your doctors where you did the medical and they could fax you the sheet. I just do not know how long they keep them. I called them 4 weeks later and they still had my file there. Just give it a try.

they'll have a duplicate copy they can send you. It'll likely cost you a few $$, but cheaper than having titres done in the US.

Wow, it feels like just yesterday we were in your shoes.

I find it unbelievable that immigration hasn't resolved this confusion yet, with the question of vacc. supp. or no vacc. supp.

My wife had her medical done in Brazil with the embassy doctor and we weren't given any copies (and my wife did ask for them, and he said you won't need them). Anyway, those sealed doc's were given to the immigration officer, which gave us sealed doc's to give to immigration at POE.

We saw the same thing about the vac. supp. when we filed I-485 and didn't know what to do. I did some research on this site and listened to a VJer, who said he wrote a letter stating that he never received any copies of his medical exam and gave the sealed docs to the immigration at POE. He didn't receive an RFE, so we decided to do the same thing.

Well, we never received an RFE, the immigration officer never asked us for the vacc. supplement, and Pamela should be receiving her GC any day now.

The vaccination supplement you get from a civil surgeon in the US. The vaccination worksheet you should have gotten at your overseas physical.

AOS paperwork asks for the supplement.

When my wife went through AOS earlier this year, we did what Andy28 did, submitted a letter explaining that all vaccinations were done in the UK as evidenced on the medical sheet submitted when she arrived in the USA. No RFE, and she now has her greencard.

Well, you can either submit AOS now with the letter saying you don't have it and wait for an RFE should they decide they need it or wait to see if you can get it. I'd personally go with the first option, it looks like others got away with out it and maybe you can too. Who knows how long it might take to get the form mailed/faxed to you so I'd go ahead and get the AOS ball rolling! :)

I've just wrote a letter telling them I had my medical, handed in envelope at POE etc so fingers crossed - in the mean time i'm getting my vaccination records from my own doctor sent over so i'm ready for the RFE! Its around $90 for a medical here in fresno as I called round yesterday and so long as i have my vac record from england they said i wouldn't need them again.....i'm trying to be prepared just in case! Hopefully we get a nice officer!!

I called the USCIS help line about this issue, and they said that if the vaccination supplement was collected at the border, that would be fine. The reason for not requiring vaccinations until the AOS stage of the process, according to the officer I spoke to, is that some countries don't have all the necessary vaccines available.

Anyway, we're hoping that writing the note will work for us, too. We'll see how it goes!

Conditions Removed! Ten-year green card received 6/24/2009. No more USCIS for a very long time.

We are in the same boat and just submitted without the vaccination sheet. We got our NOA on Saturday and a biometrics appointment for next Tues (a week from tomorrow), so we seem to be on the fast track -- though we're going to the Civil Surgeon on Monday just to be safe. We're waiting for an RFE, but if one doesn't come, brilliant.

Hi Sammy,

If you get the form 3025 from the UK Dr's that did your medical and Vaccinations and take that along to a Board Surgeon here in the USA they can transpose the info from that to the Form 693 Suppliment that they require here but they will charge you for this service, some even insist you have the full medical again so you have to enquire to one that will be willing to just see your form 3025 and transpose the info to the 693 Suppliment form as the USCIS dont use the form 3025 and mostoly always insist on the 693 Suppliment when filing for AOS EAD AND AP ...Hope this helps you out..
