Not required anymore.

"Q. I heard that the vaccine against herpes zoster (zoster) and the Human Papillomavirus (HPV) are required vaccines. Is this true?

A. From Aug. 1, 2008, until Dec. 13, 2009, the zoster and the HPV were required vaccines for immigration purposes. However, the zoster vaccine was not available from Aug. 1, 2008, through Dec. 13, 2009, and USCIS posted a message on the Web to inform civil surgeons they could annotate the vaccination record with "not available" if they were not able to obtain the vaccine. In 2009, CDC changed the vaccination requirements based on ACIP's recommendations. As of Dec. 14, 2009, the zoster and the HPV vaccine were no longer required."

Yes but I saw this too:

http://islamabad.usembassy.gov/medical_exam.html

"Please note that as of June 1, 2009, all female applicants between 11 and 26 years of age require the HPV vaccination at an additional cost of 9,000 rupees."

Maybe their website is out of date? But it is marked: Feb 22, 2010!

will the female applicate possibly be older than 26 at the time of her medical? If so...then 100% it would no longer be req'd (not sayign that it still is BUT it is age dependant adn the OP's age was not mentionned).

i am against this vaccine for those who choose not to have it or for those it was an unnecessary vaccine so was glad to see they removed this requirement.

http://www.now.org/issues/diverse/nciwr/100108immigrationhpv.html (NOW article from 9/29/2008)

i would print these out and take with you/spouse to medical exam... highlite ahead of time the important parts

http://www.cdc.gov/immigrantrefugeehealth/pdf/2009-vaccination-technical-instructions.pdf (see page 5 for HPV, page 11 for varicella, and page 16 is chart of required vaccines)

if you gave your info (receipt #s, full name, etc) to anyone on VJ under the guise that they would "help" you through the immigration journey with his inside contacts (like his sister at USCIS) ... please contact OLUInquiries@dhs.gov, and go to http://www.whitehouse.gov/contact to report anything suspicious. Contact your congressman and senator's offices as well.
