my second question is for anyone who has delt with the us embassy in manila:

For your first question, yes. However, if in case you forgot to fill out everything online, you can just use a pen with blank ink to fill out the remaining boxes. You can attach your photo before you go to the embassy.

Second question, another yes. It is a must. You don't have to attach it to the form. In my case, my CENOMAR was in a separate folder with my birth certificate and NBI.

Hmm, not really sure why, but once you fill it out online, you need to print it right away cuz I think you cannot save a filled out form of this. You can fill out the DS-157 using a pen with a blank ink though.

DS 156 MUST BE FILLED OUT ONLINE .. it will give you a barcode that you will take to the embassy

this is what it says on the website

DS 156 - This form must be completed electronically (no handwritten copies will be accepted) from the website http://evisaforms.state.gov and printed out.

i'm referring to the ds-156..there doesn't seem to be any logical reason that this must be filled

out online

What I did is download the DS156, had my fiancee fill it in, email it to me, then I filled it in online.. Made it easier for both of us... I suppose, they created the electronic form for a reason. not sure why, but that seems to indicate they want that particular form completed online..
