Hello =)

I'm currently finishing off my AoS papers (came in to the US on a K1 visa) and a recently had the parts of my I-693 filled out by a civil surgeon however they didn't put all of the paper work in the sealed envelope & I was just wondering if this is indeed normal, or if I'll need to go back to the doctors and ask them to put the separate sheets inside the envelope as well.

I'm assuming they made a mistake, but wanted to double check on here first.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

Both sections should be in the sealed envelope.

Yes. The i-693 you send with your AOS package needs to be sealed in an envelope by the CS's office. They sign or stamp their name over the sealed envelope flap border and cover it with scotch tape to make a quickie untamperable seal. You will probably get an RFE for the I-693 if it isn't sealed in an equivalent way. You should take it back and have them seal it. Maybe make a photocopy for your own interest first though.

It's great for them to give you your own copies for your own records, but the sealed envelope needs to contain all parts of the I-693, even if a bunch of them are left blank.

"Q: If an applicant is required to receive only the vaccinations, and not undergo the entire medical exam, does he/she have to submit the entire Form I-693?

No. Applicants who are not required to have the entire medical examination need to submit only pages one, three and five of the new form. The required sections include Part 1, Information About You, the vaccination record portion of Part 2, and Part 5 (if the vaccinations are administered by a civil surgeon) or Part 6 (if the vaccinations are administered by a local health department). Pages two and four, that would have been left blank, do not need to be submitted."

Also, depending on your interpretation here: http://www.uscis.gov/files/form/i-693instr.pdf in the I-693 instructions on page 4, FAQ2, C, B:

"...in this case, you are required to submit Part 1, Information about you, Part 2, the vaccination chart and, Part 5, the Civil Surgeons Certification" of course that's open to interpretation of the word "submit" where it should be "you are required to complete" if it meant that the whole form needed to be submitted. In either event the first link proves only 3 pages.

--- i only sent the 3 pages... Approved no RFE's no interview.

I had part 1 filled in by them, and had my vaccinations transcribed to the 693. But for whatever reason they left that part separate from the envelope.

Just to confirm, WHAT is in the envelope? Did they give you an envelope at all? Maybe they've given you copies of what's in there but aren't comfortable giving you the signed page as a copy??

If you're unsure call and ask them, or go in and ask them to open and show you what's inside the envelope then give you another envelope and re-seal it. Or you can just trust that it's right.

The WHOLE form isn't needed if you just got the transcription. I'm leaning towards the envelope contains only 3 pages (like it should if it's just a transcription) and is fine and you're simply confused because they gave you a copy of what's inside (sans the signature page).. they're SUPPOSED to give you a copy anyway.

The Instructions on the I-693 specificaly state.."If the vaccination part was incomplete,you must get a civil surgeon to give you the proper vaccinations and transcribe them onto a I-693 form..It states.."If done right the first time there is no neede to get a civil surgeon to do it a second time,that is clearly what it says on the instruction manual...even though we went to a civil surgeon and spend a hundred bucks for him to transfer the information for us...A lot of people including myself don't think it was needed..This is a very grey area....years back people were allowed in without all the vaccination shots,thats why the civil surgeon was needed,nowadays you have to have all your shots before being allowed to enter on a K-1...

K visa applicants are still allowed a visa and also to enter the US without getting any immunization. They will not be allow to adjust status to become a permanent resident until they meet the immunization requirements. Some make an effort to get the needed immunizations prior to their visa medical exam so everything needed is recorded on the DS-3025. Most don't make the effort and need further immunizations in the US, which have to be "certified" basically by a USCIS approved doctor aka civil surgeon. And since the list of immunizations is an American list, not all the vaccines are widely available in every foreign country.
