DS=3025 Vaccination Record Incomplete Checked

My wife's DS-3025 Vaccination Record is checked Incomplete. It shows for Tdap (insufficient time interval) for Varicella (not routinely available), measles and mumps are both blank, no check it was given or not required.

Our plan was to just have the Civil Surgeon transcribe the I-693 with no other physical needed. But now do we need to make sure we have the other missing vacinations ? Tdap (i'm guessing second round ?, and measles and mumps ?

Since our DS-3025 is checked as incomplete we will just be including a copy of that as it is with the I=693 vaccination supplement from the civil surgeon ?

Mine also they same as your record vaccination in st.lukes.I go in civil surgeon doctor,they will tell you what you will need or you need to have vaccination shots again to complete..Its depends the civil surgeon,For me is i have two shots vaccination again here i went to public health and show my vaccination record in st lukes so that we can save.And then i back to civil surgeon i had tb skin test here,i dont know why i need that.i tell them that i have done all the xray in the philippines,but they will not accept the xray in the philippines.So that the civil surgeon could sign the I-693...i had tb skin test and two vaccination shots here in the US....

its up to you if you want to transfer the vaccination record to civil surgeon,,,Coz thers also people who approved thier green card that only the vaccination record in St.lukes.It depends the person who can recieve in the USCIS some accepted some were not and had RFE...For me to make sure i transfer my vaccination record to civil surgeon......

same as mine.i read something that if the surgeon will ask you to do the medical all over again just tell him that you've done it already and you just need the I-693 for him to transfer your vaccination record and sign it.goodluck.maybe it depends on the surgeons.some just wants money.we're going to the surgeon nextweek and lets see what happens.

Where are you in Cali? The Civil Surgeon who transcribed my vaccine records only charge $ 30 in Santa Ana, CA but I have to take varicela and another shot of Td as per her. If you are around this area maybe you can check them out 714 444 4448 Dr. Lohiya. Get your vaccines completed at the health clinic before going to civil surgeon this will save you $.

Part of the civil surgeon's purpose is to advise you what shots you stlll need for AOS. He may give them himself or send you to the health dept or another clinic.

You don't need to send the DS-3025 with AOS. It is used (in the case of incomplete vaccinations) to show the civil surgeon proof of anything you had by the time of your visa medical. The complete record will be put on the I-693. Complete meaning previous shots + new shots all on one form.

No, that is not an option in the case of lbounds, who asked the question. He can't decided to skip the Civil Surgeon and I-693. He clearly knows some shots need to be taken or his wife will not be given AOS approval. He said that she is not complete on Tdap (needs another one), Varicella, and MMR. Only people who have all those vaccinations recorded on their DS-3025 get approvals without the I-693. If even one shot is missing and needs to be given in the US, then everything (old + new shots) must be put on the I-693 by a civil surgeon.

Thanks very much for your helpful replies. It cleared up my questions. I did read through the FAQ earlier. We have a civil surgeon in mind but we will go do the incomplete vaccinations first before our appointment with the CS to complete the paperwork :)

Ours was checked incomplete in Jamaica, but we only paid to have it transcribed for 35.00 and the civil surgeon checked complete on the I-693. No shots....

Let's try to verify if the varicella shots are really necessary to complete. I have heard anecdotal reports that it is NOT necessary to complete the varicella series for the civil surgeon. Like a jackass, I coughed up $125 bucks to get them done just prior to the civil surgeon. I really think one doesn't have to stress about getting the varicella shots done ASAP, prior to marriage and getting your lady on your company insurance plan prior before marriage.
