Timeline
Just getting the AOS packet together and we can't find the I-693 Vaccination form. :( We tried calling her doctor in Bogota where the vaccines were administered but they don't have a copy of the report.

What can we do?? :help:

If you have lost it you might have to do the medical again unfortunately. Ring one of the cival surgeons from the USCIS list.

I-751 - Removal of conditions

10-20-2008 - Sent package via usps - overnite mail

10-22-2008 - Package received and signed.

10-28-2008 - Cheque cashed

12-02-2008 - Biometrics in Boston

10-03-2009 - Transferred to CSV

10-04-2009 - 10 year GC on the way

You shouldn't have to do the whole medical again - do you have any record of her vaccinations at all?

If not and none can be gotten she might have to have the vaccinations again, then get the vac. supplement filled in by a civil surgeon, but not the whole medical.

AOS INTERVIEW SCHEDULED FOR AUGUST 8, 2006

AOS approved - August 8th, 2006 - hooray - done with USCIS for now!

GREENCARD RECEIVED - August 18, 2006

I thought all that was sent to the embassy when the airport took the sealed envelope? We just filed aos and we didnt put any of the medical just the cop[ies he had and sent them with the aos. I hope that is good enough

I-693 is a medical, I-693A is the vaccination supplement filled out by a US Civil Surgeon. Do you have a copy of the DS-3025, the vaccination worksheet from Bogata?

Sept 29 - green card arrives, done until June 2008 day 140-total days 381

We didn't have any copies of the medical since they were sealed and given to the immigration at the POE.

We wrote a letter stating what happened and Pamela didn't receive an RFE. I don't know whether this is acceptable, or we were just lucky, but Pamela was approved for her Perm Res last week.

3/23/06---We got married!!!!yeh!!!!!

4/11/06---Our first NOA for the I-485 and I-765

5/20/06---Our green card interview was scheduled for 7/28/06

6/03/06---We moved to Phoenix, AZ and had to cancel our interview

7/15/06---Our rescheduled green card interview (WOW! Only 10 days after our initial 1st interview)

8/07/06---Green Card Approved!!!! (Now only the wait by mail)

8/14/06---Received "Welcome to the USA" letter in the mail.

8/18/06---YEEEEEEEEEESSSSSSSS! Received Green Card today!(actually it should be called

the White Card, ha)....immigration free for 2 years!!

Thanks for all the replies...we're going to write a letter of explanation. No point in spending $300 on new vaccinations unless we really need it. Wish us luck!
