LOL! The baby is doing great growing very nicely and finally behaving her self by not make me sick much :) Thanks for asking. how is your baby doing? Its funny cause alot of the people in my office keep naming her all kinds of strange names so far my favorite is Kambresha! LOL

Hey sugar.. if you look on the paper that your hubby got from the medical with the vaccinations.. THAT is the DS-3025

Hey Tee,

I hope they don't make me scream. He never got back a paper when he was leaving the office...The only gave him the sealed medical enevlope.

He did however get his shots and they did however update his immunzation yellow record book.

I hope I dont really need it and that the i693 medical sheet filled out by the civil surgeon is good.

as long as they updated his personal shot record, it should be okay. Same thing happened to Eric. Our doctor used his personal shot record card. Wasn't a problem at all.
