I-693 vs. DS-3025 and vaccination history

Just a quick question to you guys. Reading AOS guide I'm not too sure now: should I bring DS-3025 and vaccination history to Civil Surgeon and send to USCIS I-693 only, as I thought before, or should I rather send to USCIS all three of them (I-693, DS-3025 and vaccination history)?

I-129F Sent : 2009-10-09

I-129F NOA1 : 2009-10-13

I-129F NOA2 : 2010-01-22

NVC Received : 2010-02-02

NVC Left : 2010-02-05

Consulate Received : 2010-02-12

Packet 3 Received : 2010-03-16

Packet 3 Sent : By current rules in Russia, packet 3 should not be sent, but rather brought to the interview.

Packet 4 Received : 2010-03-16

Interview Date : 2010-04-08

Interview Result : Approved

Visa Received : 2010-04-14

The I-693 should be sufficient. Possibly including three things effectively saying the same thing might confuse them.

send the I-693 and the DS-3025. The DS-3025 was sent (for me) to prove that a medical was done for the K1 and at what date.

from my understanding the i693 is not required?

N-400:

02/21/2015: N-400 sent via USPS Priority Mail to Phoenix, AZ Lockbox

02/23/2015: N-400 received by USCIS

02/27/2015: Check cashed

03/03/2015: I-797C, Notice of Action received

03/26/2015: Fingerprints done!

08/24/2015: Notice of Action received, Interview letter!

09/28/2015: Interview day, PASSED! Oath Ceremony same day :)

from my understanding the i693 is not required?

It is if your DS-3025 is marked "incomplete".
