Wife and I are sending i751 this week. The package is complete; however, one question still plagues us - the Last Name. Her GC shows her MAIDEN name. We are sending it in with her MARRIED name, and we stuck her MAIDEN name in the MIDDLE name field. Any thoughts? The application reads: FIRST / MIDDLE & MAIDEN / MARRIED LAST; the expiring GC shows: FIRST / MIDDLE / MAIDEN.

Are we reading different forms? Part 1 (Information about You) reads third on the first line "Full Middle Name" saying nothing about "Maiden name" there. However, if you now turn to Part 3 Question 1, it says "Other Names Used (INCLUDING MAIDEN NAME)". So, I would simply put her FIRST / MIDDLE / MARRIED LAST in Part 1 and put her MAIDEN LAST in Part 3 Question 1.

Ack ... I knew I would mess something up :blush: I didn't change the number of CSV applicants by mistake :whistle: . There should now be 13, including me, and not 12 that is stated.

Please the next person who adds their name can you simply change the number of CSV applicants to 14 instead of 13? Anyway, good luck to all! :thumbs:

So I just started on our I751 paperwork, only to think how easy this is - just fill out the 3 page form and you're good to go! Needless to say, I forgot that they need a whole pile of other stuff to prove it's not a sham! Darnit, I thought they simplified the process!

Oh well, time to start finding things to give them....

I've been tracking my package sent on August 11th via DHL and just now it says: Delivered - Signed for by : A STANLEY Boston, MA - USA ????

I've called DHL here in Switzerland and they said they'll open up a trace and get back to me tomorrow. Oh God, I hope the package arrived at ST. ALBANS, VT otherwise I'll be screwed as my wife's GC expires next week.

Anybody with similar experience ?

I think this is an intermediate point for DHL. I don't think they meant "final delivery location". If you sent it to St. Albans, then they will deliver it to St. Albans, VT.

v333k, I really hope what you stated is true but the fact that it mentions name of person who received the package in Boston is quite worrisome :(

I realize that it is very concerning what you see, but I am wondering since this is an international package, a DHL person needs to sign off on it to maintain records of international-shipping... I guess these rules exist to make sure nothing threatens the US.

Hope you see another update tonight (or tomorrow) with a St. Albans location.

I'm having a similar problem. they tryed to deliver my package yesturday (sunday) and what do ya know, nobody was there to accept it. so they left a notice and have 15days to pick it up or its returned.... i really hope they pick this up. my husbands visa expires sept. 11th :(

I sent my paperwork via usps and had it certified..when I tracked it online, I did see the name of the person that signed for it at the destination (not sure how much my reply helps..just wanted to say that I saw a name online as well)

Very good point. I took your advice and just dropped the whole keep-the-maiden-name idea. K.I.S.S.: Keep it simple stupid. Middle name means simply that. Thanks for your input. Mailed i751 today express mail to VSC.
