We just recieved our packet on July 28th. My fiance is a little nervous cause he is a stickler of doing things. It has to be right. He wants to know if he can use black or blue ink pen to fill out the forms. Or must they be typed in BOLD letters, as it says. Are both forms DS156 and DS156K suppose to be in the packet and where would he get the DS156K or is that an online process? Should he ask for a DS3025 when he gets a medical done if the shots run after the interview date to prove that he is in process of vaccinations? It says for him to mail or take back the forms after they are complete, but I told him to wait for his interview and take them at that time. If any one has gone through this, can you please shed some light on this experience. All will be very Appreciated and thank you in advance. Good luck everyone and God bless.

Accra you should bring the forms with you! I saw it said to mail something in to get your interview date. I think they have combined some old things and new things- almost all the papers are very old- they need to update things.

I think thats why he is a little confused because of the old and new forms or papers mixed together. He just wants to make sure that every thing is right. The Ds-3025 form is from the doctors in case the vaccines are delayed after the interview date. From what I understand, the form is to prove that the shots are in process. I read it somewhere on Vj. Just checking and trying to be informed.

We had the same confusion with the forms that say you should return them right away. My fiance actually took his back to the embassy the next week to check, and they wouldn't take it. They just told him to bring everything with him to the interview.

11/24/2010 - 3rd appt.--Finally have all documents in. Result-Administrative Processing! Consulate says they will contact my fiance

05/18/2011 - Admin processing complete! Babs is asked to appear at the embassy on the 25th.

05/25/2011 - En route to the embassy, Babs is robbed on the roadside. His passport and other papers are all stolen. After filling out a police report, he arrives at the embassy a little late, and they allow him in to speak to the CO, even though he has no valid ID. Thank God! The CO gives him another appointment on June 15th, to bring a new passport and also an updated police certificate.

06/14/2011 - New passport (expedited) ready.

06/15/2011 - The CO finds a missing stamp on the new passport. Sends Babs back to the passport office and tells him to return in the afternoon. All is taken care of. VISA APPROVED!!!! PRAISE GOD!!!!

06/24/2011 - Visa pick up date--told to return on the 30th.

06/30/2011 - Babs returns but doesn't get the visa. Back in AP!

10/05/2011 - Embassy brings Babs back with updated medical. Approved!! CO sets pick-up for only 2 days away, so as not to delay him any longer.

10/07/2011 - VISA IN HAND!!

Jen- it does say to return one in the packet- but I figured it was incorrect because it said to get your interview date please return... we already have it!!! Oh Ghana!

My question is when your fiance went for the medicals did the clinic have copies of the forms that the embassy requires? We were not given the DS-3025 forms i belive is the number- and they really arnt online for printing- not the whole packet at least.....

Also do you know what the total was for his medical? I know it varies slightly (no. of vaccines required) but I want to make sure Israel has what he needs when he goes. Which place did he go?

My fiancé hasn't actually gone for his medical yet. He lives in Tamale, so he is planning to go back to Accra and take care of all that at the end of September. That will give us a month to do the medical and get the vaccinations, which I hope will be enough time.

I wasn't aware that there are any other forms he will need. Looking at his packet, it just states to bring the results from the clinic in a sealed envelope to the interview, and doesn't say anything about a form that needs to be presented at the clinic (it just says to bring a passport and three passport photos). He'll probably go to Akai House or Holy Trinity since they are listed as providing all the necessary services.

It just occurred to me that I could call the clinics and ask--their numbers are listed. It would be a lot quicker than waiting several months for the embassy to reply to an email. I'll let you know if I find out anything.

My fiance called this morning- they have the paperwork- or at least they say they do. But they didnt give it to any of us so that makes sense. (at least at the Akai house- I would assume Holy Trinity have them as well...)

The quoted him for 380cedis for everything at the Akai house but not does not include the vaccinations. They are 20cedis each- and the number required vary depending on the person. From what it sounds like you pick up the results 3-4 weeks after, and you do need to get a 2nd shot a month later/maybe same trip as pickup!?!?! Its probabally cheaper in Ghana so if you can get it before travel its better- and then there is one more 6months or a year later, which would be here (hopefully!).

A fellow member let me know that the vaccination record should be given to you- in an unsealed envolope so when you go back you can have them add additional vaccinations. I would suggest for AOS purposes you make a copy as soon as you get it (and with any additional shots)- so it cant be lost in the brown envolop at POE! I have seen members who had issues with this and to get a new copy is very hard. The test results will be sealed and brought to the interview along with the vaccination form.

Also Ghana1821 and fiance let me know that you can NOT get your police certificate more then a month before the interview- and it takes two weeks to get. So I am having Israel request it exactly one month to the interview so that if its not there the first time around then he can re-request it with enough time. The price in the past has been 20cedis with maybe a little dash???

Ghana1821's fiance also said USD is a little better for VISA ($350) fees because they give a really bad exchange rate for Cedis at the embassy.

I would definatly make sure you get a reciept for all of these services-

Great! I called this morning too and got basically the same information you had posted (also from Akai House).

We may have to get a second police report. It's supposed to be 20 ghana cedis. My fiance's was extra expensive because a friend of a friend was there and took him straight back to the office and then quoted a huge price to him. Babs didn't know the actual price so he didn't realize he was just paying a really big dash. So watch for that.

Thanks for the info about the police. I will see what Israel deals with later. I would get another one if yours is going to be over a month old. People with more then one interview have to get a new one each time. Better safe then having to wait for another interview!

I am interested to see if the 380 includes or does not include vaccines.... I told israel to bring extra to cover the vaccines just in case.

Thank you. For what Im understanding that 380 cedis is for the medical and not sure if vaccines are included, 20 cedis is for the police certificate and has to be less then a month old. This is what I had read that the DS-3025 is a form for when the vaccines run its course past the interview time. Thats to show the Embassy that the vaccines are in progress and will be completed before leaving Ghana. If the shots are not done before the interview, that will be a way to show proof of the progress. You may have to ask for it if you think that all the shots will not be completed by the interview.

Thats what my understanding is to. I believe you are able to keep the vaccination form but the test results go in the mystery brown folder so that you can get more shots as needed. I am gonna have Israel ask when he goes- but I know they don't seal it- its more do they take it at the interview or do you get to keep it? I hope you can keep it- but definatly make a copy so for AOS you have something- I just read another post of medical forms being lost....

Im definitely tell Evans to make copies of the medical just to be safe. I would hate to go through all that and they lose the forms. I should ask if they will make copies at the Holy Trinity or Akai House, that way the doc can sign both.

I dont think the medical exam and results you can because they seal them- but its worth asking. When is Evans going for his exams?

I will let you know how Israel does Friday and if they can make an extra copy of the results.... but he wont be picking them up for a few weeks anyway.

He is not sure when he is going but Im trying to convince him to go early in case that vaccines take some time to do. Hopefully soon.
