and scroll down to Lifting Conditions (I-751) Related Forms there are some examples of sworn affidavits. It shows name, address, birtplace, and date of birth as requirements.

Hi..Thanks.. i looked this already. did you have yours notarized? We just made letters and have friends signed them but maybe at least one should be notarized.. Glad you gonna have your biometrics soon. good luck.. :thumbs:

I had two affidavits from family notarized and the third affidvit from a friend wasn't notarized. The notarization was done at our bank for free. I believe the notarization gives value to the affidavits because it verifies that the notary has assured the identity of the individuals signing the document.

Thanks. It is a two-hour drive one way to the biometrics. But we plan to take advantage of the Japenese Buffet near the Application Center.

Naturalization

3/23/14 - N400 package sent to Phoenix

3/27/14 - N400 package delivered

4/3/14 - NOA1 receipt date

4/4/14 - check cashed

04/29/14 - biometrics date

07/01/14 - interview date

xx/xx/xx - Oath Ceremony
