Do I need to send I-693 if I have DS-3053, 3026, 3024, 3025?

I have the following medical form copies.

DS-2053,DS-3026, DS-3024, DS,3025 and chest Xray

Do I still need to send I-693 and have it signed by a civil surgeon?

The instructions i have read elsewhere says that I need to still have the form filled out by the surgeon but the Q&A sticky on this forum says otherwise..

Those medical forms (originals) are supposed to be sealed in the brown envelope you turned in at POE. So if USCIS does their job properly, they will order your file to match up with your AOS when they receive your application. They will have your medical results.

The key to what you need is to know if all the required immunizations were received before/during the visa medical and recorded on the DS3025. A K1 visa can be granted without meeting the immunization requirements, so it's very possible more shots are needed in order to adjust status. A civil surgeon will look at the record and get your immunizations up to date. You don't need all the bloodwork and TB testing, just the immunization page filled out and of course page one with your info and the last page where the civil surgeon signs off on it. Some people who are positive that they have every shot needed for AOS recorded on their DS3025 submit that and are approved without going to a CS. If in doubt go to a CS. Call ahead and get prices because some charge way too much. Don't get another full exam. Just get the immunization section looked at and completed.

Should I look at the Dept of State vaccinations list and compare it to what's listed on my DS-3025?

Is that considered an accurate list to compare against? If so, do I need to have ALL those vaccines administered for AOS?

The list comes from the Center for Disease Control (CDC) which is under Dept of Health and Human Services, not Dept of State. Most on the list are not age appropriate for adults, so you probably have that column ticked off on the DS3025 for the ones you don't need.

Having chickenpox disease eliminates needing Varicella, but it would have to have been recorded as VH (varicella history) in the column beside Varicella by the panel physician. If it isn't then need the shot or need to get it straight with a civil surgeon.

Most adults need only MMR and Tdap (or Td) and Varicella (or history of chickenpox)

Note on the chart: It still lists HPV for women of a certain age group because CDC still thinks it's a good idea, BUT it has been removed as a requirement for AOS, so ignore HPV.

Some people will say that your DS-3025 must have "complete" checked, but that's not true. From personal experience the AOS was approved with the box checked that says incomplete, blanket waiver.......

You get a "waiver" for any shots not age appropriate or medically appropriate. It's basically automatic. I've read the entire instructions to panel physicians and to civil surgeons and if you want to look it up on CDC website, you will see in the instructions to the panel physicians, it says the box "incomplete...blanket waiver...." will almost always be checked. There shouldn't be any blanks on the DS-3025. It must have a date of shot, VH, age not appropriate.....something besides an empty line.

So how safe would it be to assume that if all the vaccinations have something written(or checked) in front of them, then I am good to go?

I think I am okay and don't need to cough up more money for medical but I am also trying to weigh it against a RFE...

We had the same question when we filed my AOS and we were advised to just send the DS-3025 and not the I-693. We were lucky that they accepted it without giving RFE and without spending money for the civil surgeon.

It's just something you've got to decide since nobody is looking at your immunization sheet but you. Does it have a recent (not childhood) date for MMR, Tdap or Td, and Varicella or VH written on the line? If so, that's a pretty good indication that all are there. And will your world crash in with an RFE?? Mine wouldn't since I was married and living under the same roof with my fiance/husband. A possible couple of weeks for an RFE didn't scare me at all. We didn't have any deadlines to meet. But each couple is different.

I hope that stuff you have is just a spare copy of the medical exam and the originals are in your brown envelope turned in at POE. If they don't match your medical up with your AOS application, then they do send an RFE. Lucky you with a copy of the whole medical. I think I would photocopy all of that and just include it for the heck of it since you have it if you decide to not do an I-693.
