quick question guys, on form ds-156, #17 is asking about marital status..my fiancee was prevoiusly married and it is now annulled..but #17 question is asking regarding her marital status..the choices listed are..1,single(never married)..2, divorced..3, widowed..4, married..5, separated..so which one should she choose..and also for question #8-9 is asking for previous surname and middle name used..should she put the prevoius names that she used even if her marriage is already annulled..

yes all previous names used in the past... meaning include her name her previous marriage.. check the box says single...

thanks for your quick reply..im not doubting your answer but, for the single answer, it is enclosed with parenthesis saying "never married"..so, can i still choose that answer still..
