Yes there is a fee. The fee is widely different depending on the civil surgeon. Most of them charge outragious prices. Search around the civil surgeons in your area and see how much they charge and go with the lesser priced one.

I'm going through this process as we speak. Definitely make multiple calls. I was quoted anywhere from $75 to $140. Make sure you tell them you don't need any lab work (blood tests, xray, etc), just the form filled out. There is no set rate, so all of the doctors can determine to what degree they want to rip you off. Sigh.

I know a girl who just started her AOS she is not on visajourney we just met on the plane in march and have kept in touch. She jsut filed her AOS papers and took them to the USCIS office in portland for them to review before she sent them off and they didn't tell her she needed the I693 form complete. She sent her AOS papers off without having it completed she said she never heard of this or was never told by her local USCIS office. I suspect she may get an RFE then? So to be clear we definatly need to have a civil surgeon complete the form for us when we adjust our status regardelss of when we got our medical done even if its just to transfer information no lab or blood work or xrays.

Some people get away with it when they send in their vaccination form, and some don't. Depends on your level of ability to accept risk...if she isn't adjusting from a K1/K3, she needs a full medical, actually.

We paid 25 dollars only. To me it was worth it to avoid a possible RFE.

After sending my AOS papers, I got an RFE saying the USCIS had "no records of my medicals" (which is impossible because I handed the envelope with everything in it at POE). What happened was that the Immigration misplaced the report the doctor in Brazil had completed in November.

So I had to go to the doctor again. He accepted my blood exams and X-ray but said my vaccinations were not complete ( ). Oh, and said I needed to do a skin test for Tuberculosis because the X-ray was not enough (I was like "What??"...)

At that time I was living in a very remote place so didn't have many options of doctors to choose from... Ended up paying $300 for the appointment + $170 for the shots. :crying:

So, yes, if you have a choice, call all the doctors you can before picking up one.

Good luck!

Lu

Edited to add that he wouldn't only fill out the report. He said he couldn't do that. He had to interview me and "check me" again (luckily no physical exam...... Only weight, height, temperature, breathing, etc. )

Not really that impossible when you think about how many files and packages cross their hands/ desks etc every day.. its probably somewhere in the office but they just don't feel like looking for it. That and some POEs never forward mail or take forever to forward it. Iv seen some people post about POEs never mailing ahead the packages and losing them in the POE.

I suggest if you didn't keep a copy of your DS-3025?? (vaccination records) you could try to email the hospital where you had the medical done and request a copy. I had a copy of my husbands, but out of curiosity I emailed MoBay Hope which administered his medical and they emailed me a copy.... If I remember correctly the form is made in triplicate and a copy is supposed to be given, but a lot of times it is not.
