Se achar um médico que aceite sua carteira de vacinação em português, tudo bem. O problema é justamente esse.

Eu peguei cópia do meu DS-3025, então pra mim foi mais fácil.

I told you I had the records I got from the Posto de Saude before I went to the doctor.

And after I got back from the medical, I told you the secretary gave me a simple copy of the form that is in the not-to-open envelope.

My question is: Is a simple xerox of the DS-WHOCARESWHATSTHENUMBER form enough for the AOS?

yes, provided you get it transcribed onto an i-693a vaccination supplement. if you need assitance in finding a civil surgeon that is uscis approved, alex, pm me.

and it's a ds-3025 rey :P

PS It would really help if you would stop living in a country 4 hours ahead of Minnesota so that when I am up late worrying you are actually awake to answer my questions, and not just waking up for work ;)

Rey, é isso mesmo que vc vai precisar mostrar pro médico aqui pra pegar o I-693A e Lisz, normalmente os médicos não dão isso mesmo não, só se vc pedir e alguns só se vc pedir com insistência.

Só uma pergunta....se vocês estão casados e são dependentes dos maridos/esposas no plano de saúde, as vacinas não são de graça? Mas acho que isso só vale prá quem não tem esse papel do médico brasileiro, ou prá quem tiver que tomar o restante das doses aqui (como eu vou ter que tomar a última de Hepatite B ).
