Hey guys. I posted this in the AOS forum but I'm wondering what can be done. Rey did not get an I-163A form at the medical AFAIK, but he just got a sheet of paper in Portuguese with the vaccinations on it. What should he do before leaving Brazil so that he doesn't have to go to yet another medical?

Luke está preocupado pq não sabe se conseguirá sair da cidade na quarta-feira para vir para nossa entrevista. A cidade onde ele mora, Tulsa, está sofrendo com chuvas de gelo. Vários vôos cancelados neste fim de semana. Ele precisa ir para Houston, se for de carro serão quase 15 horas dirigindo numa estrada perigosa. Muita gente já morreu em acidentes por causa do tempo. A previsão do tempo anúncia nova tempestade para hoje.

alex, that is probably the ds-3025 form. probably best to get it translated while in brazil. once he gets to the states he takes that to get transcribed onto an i-163a vaccination supplement form. it cost nessa and i about $40 to do that. you'll need to find a uscis approved civil surgeon in your area for this.

Yes, when I had the medical in Brazil she sent me the sealed envelope and she gave me a copy of the DS-3025 for myself (that is part of the papers that's in the sealed envelope I took to the interview with me). The DS-3025 is in english, it says all the vaccines I've had. Once he's in the States he can schedule an appointment with a civil surgeon that will transcribe the DS-3025 onto an I-163a that he will send with his AOS package when you start the AOS process.

eu to ainda enrolada com esse caso de suplemento de vacinas... eu arrisquei e mandei meu AOS sem isso.. eu sei.. mais depois de conversar com algumas pessoas e ver que seria 50 / 50 de chances de dar certo, resolvi arriscar. The thing is.. eu nao tenho esse DS 3025.. se eu for la no medico so com a minha carteirinha de vacinas, ele pode fazer a transferencia para esse 693A ? senao.. eu to f......
