I have all the approvals so far, NVC sent the packet to Manila. My fiance worked in 2001 and went to japan twice 6 months at a time, once in 2004 and once in 2005 through an agency. She has no documentation and can not remember the exact dates and the addresses of where she worked. What should we do about this, how to put on the ds-158? I don't want to omit it if it will result in a denial. The job in 2001 was in another city in production. We might be able to find all the dates for this, she still has the ID for it.

The forms ask if she has been to another country in the last 10 years, and she has, and asks for employment history...

Wouldn't the stamps in her passport help in remembering the employment dates? Also, contacting the agency that placed her in those positions could be of help.

Wouldn't the stamps in her passport help in remembering the employment dates? Also, contacting the agency that placed her in those positions could be of help.

The agency is no longer around, and never gave her the passport. The agency kept it and now is closed.Pretty normal in dealing with an agency. Her passport is new now. Who should she contact to find the dates gone and come back?

Addendum to the form with the as much information as you can provide I would think would be sufficient. While the agency is not around, there is a record somewhere of the agency. Note to the best of your/her recollection the names and dates and what she was doing with them at the time.

I think that would be enough.

Thanks, yes that is what I have been thinking to do. The listing of accredited agencies list them as not available, delisted. I will add paper explaining and print delisting notification. It has been a while and I think it will be ok, too.
