hello vj's

i have a question... how do i fill in the DS3032 FORM (COA)for my 2 years old? the agent will be my husband

and im also sending EMAIL to NVC

here is the example of the template

And her visa case number is # MNL###########.

Per the DS-3032 (Choice of Address and Agent for Immigrant Visa Applicants), I would like to appoint My Husband as her agent of choice to receive all future correspondence from the U.S. Department of State. His contact information is provided below:

All mail from the U.S. Department of State concerning her immigrant visa should be sent to the address above.

I think it looks great. But where you say you appoint your husband~put I appoint my husband, HIS NAME, as her agent of .....

Also when you email it, make sure you put the case number in the subject line!!!
