My lady received package 3 from the U.S. consulate in Guangzhou, China. It has an instruction sheet that ask to provide certain documents. Can you conform that I get this right?

Also have a question about marriage certificate. We both single and never married, do they need any documents to hand in to prove that? Please let me know if anything missing from the list below.

She will need to get these:

Get an EMS label from the China Post office. Fill in the information required on the label. (note: please write “Immigrant Visa Unit, American Consulate Guangzhou” in the sender’s Company Name box, and write “No. 1, South Shamian Street, Guangzhou 510133” in the Address box)

Make a copy of the bio page of your passport ( where the page shows you birthday and other information.)

Six full-face photographs (passport photos) with white background. Size of the picture: 50 mm square.

Birth Certificate and Police Certificate. You can get it at your local municipal or provincial notary office. (Gong Zheng Chu - 公證处) These documents have to be translated to English. They can do the translation in the notary office.

Pay 400 USD or equivalent in RMB at CITIC Bank address: 广州 北京路支行 广卫路 10 号 020-83340724. They have U.S. visa fee collection service.

Form DS-230 part one

Form I-134 – I have to fill out this form and send it to you. I will also have to send you all the other forms and documents I sent to the government.

You could certainly review what's on the GUZ IV web portal for forms - at

The OF-169 is a checklist - good luck with it all - it IS straightforward - but sometimes ppl wait a bit, until they have all documents in hand before sending it in - which is kinda wonky, IMO - as after sending it in, it's another 6 to 11 weeks before an interview date, certainly enough time for you two to finish up all documents.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !
Ya know, you can find the answer to your question with the advanced search tool, when using a PC? Ditch the handphone, come back later on a PC, and try again.

In regards to this question, the same 公證处 where she gets her notary documents they will also provide her "singleness certificate." These documents have to be acquired where her hukou is registered...so if she has been living in Shanghai, but her hukou is in Chongqing..she has to take a trip home.

Nov 6, 2009: "I had breakfast in Korea, lunch in Shanghai, and dinner in Chongqing...now I just need to find a squat toilet..."
