'C/O' means 'in care of', particularly useful if the address at which you are living is not your own - for example if you're living with a friend or a relative.

Family name should be whichever name she wants to be known by.

Quick question...just filling out the I-485 AOS form and am confused about what "Address - C/O" means...

Also, we are married but I assume that the Family name should be her maiden name, not her new married name, yes?

Thanks for the insight. She wants to put her new married name as "Family Name" however isn't she listed in the USCIS system as her maiden name? Won't this confuse things if I put her new married name?

She has been reduced to a number (her A number), so using her married name will not confuse a thing.

Not as much as trying to change her name once the application is submitted and she decides she wants her married name on her Green Card.

There is also a spot for "other names used". You can put in there her maiden name.
