We are starting to fill in the necessary documents needed for "packet 3". My SO has questions that I can't answer.

:help::unsure::help:

1) The link to the London Embassy doesn't show the "checklist" that I have read on here is necessary to send. Also the UK Wiki doesn't link to it either. Is it still required?

2) Are there any forms that are supposed to be e-mailed? I thought I read somewhere that there is form to e-mail and others to mail. Am I just going nutty? (don't answer that, lol)

3) If the checklist is required, does he need to schedule a medical before he sends it, since it asks for a date?

Sorry for all the questions, but I promise I have searched the site and seemed more confused as ever. Thanks for any help that anyone might give. I appreciated it all.

05-17-11 AOS Interview APPROVED!! "Green Card" is on it's way.
