ok another question the instructions for the I-485 says that my wife needs a vaccination supplement. Would this be the DS-3025 or she absolutely needs to see a civil surgeon. The instructions make it seem like all she needs is the DS-3025. She's been in the USA a little more than 2 months only.

Does her DS-3025 say incomplete? It's recommended to have a CS sign off on the I-693. Some have just sent in the DS-3025 and had no issues, some have received RFE's. I sent in the DS-3025 and the I-693. If you get a RFE for not having the I-693 it could delay the process a bit.

You will need to have the I-693 completed by a CS since your vaccination is incomplete. If you don't send it completed you take the chance of getting a RFE and delaying your AOS.
