1) I-693 can be found on the USCIS website (USCIS.gov) or the CDC website (CDC.gov)... Vaccinations can be received anywhere but you can ONLY go to designated USCIS civil surgeons for the I-693 to be prepared. A Civil surgeon locator can be found on the USCIS website.

3) 10-14 days typical
