K2 Letter

My daughters father sent me the following letter to be approved prior to him having a lawyer notarize it. He said he just wants to issue a travel letter as well as the consent all in one. Is this ok?

Here is the letter: (names and details altered of course)

To Whom It May Concern

1. I, Joe Blow, (father) am the natural father of:

Jane Doe Blow

Date of birth: January 1, 2003

Place of birth: Edmonton, Alberta, Canada

Birth Certificate #:xxxxx

Canadian Passport #:xxxxx

Place of Issue: Edmonton, Alberta, Canada

2. I, Joe Blow, am aware and give full permission for Lola Blow (natural mother and lawful custodial parent) to permanently relocate Jane Doe Blow (born January 1, 2003) to the United States of America.

3. I give my consent for medical assistance to administered, at the discretion of Lola Blow, should Jane Doe Blow become ill or involved in an accident.

4. Jane Doe Blow, has my consent to travel with Lola Blow (her natural mother and lawful custodial parent) between Provinces in Canada, as well as between Canada and the United States, and between the States once relocated in the United States, at her sole discretion from the date of this letter July 5, 2010, onward.
