Do Civil Surgeons Maintain Medical Records of I-693

I wanted to know if the Civil Surgeons have any records of the I-693 forms that they fill out. My dilema is that I had to undergo a the entire vaccination list approriate for my age when I went to a civil Surgeon to have my form filled out. Got 4 shots. Now I have to submit an MMR vaccination record at this college I have applied to and am not sure asking the same civil surgeon would help. I got the I-693 form sealed from the clinic and am wondering if they maintain any records or are authorized to give me proof of it if they do. Anybody has any experiece or advice?

One more question: Is MMR included in the list of shots that we get for the I-693?

It can't hurt to call and ask. We had to get ours again (thank you, RFE for requesting info that we've already sent), and they had it on file. Not sure how long they keep it though.
