Help! Interview booked - questions about checklist!

Hey there!

Just got notice that I have an interview in Sydney! Hurrah!! Just a query though, the letter says:

"Please ensure that the panel physician provides you with a copy of Form DS-3025, Vaccination Documentation Worksheet as part of your premanent health record. You should hand-carry this document with your sealed visa package and any other medical paperwork."

Ok. I got a panel physician to do the medical, but I sent everything they gave me to Sydney, and am not sure if form DS-3025 was there. I'm guessing it would have been, but I wasn't given a list of the forms to check off if they did it right! And I'm also guessing (from what I have read) that I get the sealed envelope at the interview, or after when they send my visa to me. So does this mean I will get the form DS-3025 at the interview?

Sorry! Just confused! And since I am traveling from Perth to Sydney I want to make sure I have everything I need! It's just this paragraph is up the top of the page, then below it has a checklist with passport, fee and pre-paid envelope, but no form DS-3025. So maybe I just answered my own question?!

This would be a great question for the panel physician. Suggest a postal letter enquiry.
