To those looking for civil surgeon in Dallas Fort Worth,,i would really recommend DR. SAMUEL LEE, 750 North Freeway

Fort Worth, TX 76102 ;(817) 625-8818. He really knows what he is doing in terms of immigration purposes specifically the form I-693 medical supplement. The clinic is clean and the employees are friendly. Their vaccine transcription is $35.
