Ok, I am getting set to send in my AOS packet and I just want to double, triple, quadruple check that I have everything right. I have been reading up on it all here on VJ and USCIS website.. but figured it's always best to check with those who have gone through it!

FORM I-485 FORM I-765, FORM I-130, FORM I-864, FORM I-693 -------> These forms can all be sent off concurrently to the USCIS Chicago Lockbox.

FORM I-485 Application to Register Permanent Residence or Adjust Status - Fee $930, Biometrics Fee $80

Includes:

*copies of my birth certificate

*copies of my passport and I-94

*2 x color passport photos, glossy finish with my name written on the back

FORM I-765 Application for Employment Authorization fee is included with I-485

*no additional evidence required

*Question #9 - social security number (include all numbers you have ever used if any) ------> I only have a Social Insurance Number from Canada: will this need to be included? Or only US Social Security Numbers?

FORM I-130 Petition for Alien Relative - Fee $355

Includes:

*copies of my birth certificate

*copies of my USC (sponsor) husband birth certificate

*copies of my passport and I-94

*copies of my USC (sponsor) husband passport

*2 x passport photos of my USC (sponsor) husband with name written on back

*copies of divorce certificate from previous marriage

*copies of (one or more) utility bills, joint bank info, mortgage info, etc

*copies of photographs from our relationship, proof of our relationship

*one page letter detailing our relationship, how/where we met, etc

*completed form G325A for me

*completed form G325A for USC (sponsor) husband

FORM I-864 - Fee $70 to State (not USCIS)

*Includes copies of USC (sponsor) husband W2's from past 3 years

*copies of pay stubs of USC from past 6 months

*letter from USC's employer

FORM I-693 Report of Medical Examination and Vaccination Record - Fee included with Form I-485

*I have my appointment with a civil surgeon from the USCIS appointed list for this week

*I will bring the forms with me and the doctor will complete the tests and seal forms in an envelope. I will not open this and will include it with the packet

Oops - I should make a correction and say that there is no fee of $70 to State if I send the Form I-864 concurrently with the AOS (I-485) packet. It seems the I-864 is covered in the lump sum fee of $930..??

Looks good except that I think I remember you have to send all the same proof of relationship with the I-485 as you sent with the I-130. I might be wrong.

I printed out the instruction page for each form and literally checked off each item listed in it. I robotically read aloud each form to make sure I didn't miss anything. (A lot of dates kept switching back to 2010 if I didn't go back and change them each time I opened the form.) This is how I proofread student work because I find if I read it intuitively, I subconsciously correct any errors I come across.

You are right. There is no $70 fee if you are filing an AOS.

Good luck! I wish you a smooth journey.

5/29/10 - Interview letter received 6/30 at 10:30 (Day 46)

6/30/10 - Interview: 10:30 (Day 77) APPROVED!!!

6/30/10 - EAD received in the mail

7/19/10 - GC in hand! (Day 96) .
