You don't necissarily need to send in an I-693 with your AoS package. Send in a copy of the vaccine worksheet, and keep the original. If your case is transferred and you dont' get an interview, no need to spend the money on a civil surgeon. If you do get assigned an interview, you will need to take the vaccine worksheet to a civil surgeon to have the I-693 done. I was told by our interview officer that they are all supposed to ask for the I-693 at the interview.

I never got a copy of the vaccination worksheet, I only got a copy of my titre test results that I submitted. After several e-mails to Dr. Seidens office I got confirmation that the vaccination report was included in the package.

If I was able to find a civil surgeon in the area (or within an hour or so drive of here) that would transcribe the vaccinations without having to do the full medical again, I would definitley do it to prevent any chance of RFE, but so far I haven't been able to find one. And with money getting a little tight and a baby on the way...$300 extra for something that is really not required is pretty tough to swallow!!
