I almost have my AOS package ready to send in, and the only thing I need to do is with my vaccinations. I never recieved a vaccination worksheet that others have mentioned recieving, and from what I have been reading that you can submit this (I think it is the DS-3025) with your AOS instead of the I-693. Does anyone know if the DS-3025 is part of the "do not open package" that we have to submit at the interview. I have sent several e-mails to Dr. Seidens office asking this specific question and have recieved every answer but!!! I have called around to all the civil surgeons in my area and none of them will transcribe my vaccinations without doing a full medical. I really don't want to have to pay another $300 or so dollars for nothing!!

It was not part of my 'do not open' package - It was set aside from the sealed envelope in what his office mailed to me after my medical. I did not send it in instead of the I-693, but used it when I had a civil surgeon transcribe my vaccination record for that portion of the form.

My vaccination record sheet was pinned on top of the "DO NOT OPEN" envelope and I will be sending that in with my AOS package as the AOS is well within a year of the medical exam.

I think you should try calling them and ask them for the vaccination sheet.

I would suggest that you DO NOT send in the original vaccination worksheet. If you get assigned an interview, they will ask for the I-693 which you will need the vaccination worksheet to obtain.

They should have given you a copy of the vaccination worksheet separate from the medical results- I really have no idea why they didn't.

Did you try calling Dr. Seiden to request a copy? They should still have all of your information on file. Without it, you're life is going to be a little harder. Not only do you not have the vaccination supplement in, I'm not sure that you would be able to have an I-693 filled out. I'm not sure a US civil surgeon would accept your own personal vaccination records to transcribe onto the I-693 or not...

She could still get an I-693 filled out by a civil surgeon, but she would have to get all the vaccination shots from that doctor too. :wacko:

I was just thinking that without the vaccination supplement she doesn't have anything to prove she's already had a complete medical... And civil surgeons aren't smart enough to know it was a requirement of the K-1 process. That and they look for any excuse to force you to redo the medical.

I asked Dr. Seiden's office about the DS-3025 and got this reply "We are only permitted to give you a copy of the vaccination sheet and the x-ray report."

I did get a copy of the vaccination records "Vaccination Documentation Medical Worksheet Two" and a document saying my chest x-ray was ok, but that was all. Will that be enough to get a civil surgeon to fill out the I-693?

And does anyone know if they'll charge for the service? That's the one thing I couldn't find out.

We didn't have a 3025 either. When we were doing the AOS, it took about a week, but we were able to get in contact with Dr. Seiden's office in Toronto by leaving voicemails and sending emails. They were able to fax the vaccination information, which they said was the 3025 necessary to have the vaccination supplement done. For the 693, the civil surgeon did not need the x-ray. It cost us $40 to have the 693, and it was done in an afternoon. I know we could have submitted the information Dr. Seiden faxed us, but we had the time to do the 693 while waiting for our marriage cert.

I did not have my original vaccination records, so I had all my titre tests done and submitted those for my medical at Dr. Seidens. I got a copy of those ( which I already had from my family doc who ordered the tests for me). So maybe because of that I didn't get the worksheet? I am just trying to find anyway of not having to do a medical again, or travel several hours to a civil surgeon who will just transcribe the vaccinations. I so can't wait for all this to be done!!!!

I didn't receive a copy of it at my medical in vancouver sooo what i did was i emailed them and told them to fax it to my dads office he scanned it and emailed it to me i printed it off and viola i got approved with that so i'd try and get it from them

With Medisys, They gave me: Vaccination documentation worksheet - for use with Ds-2053. I don't even know what is the Ds-2053!! The little case "vaccine history complete for each vaccine, all requirements met (documentated above)" is checked, i guess i'll b ok with that, to have it transcribe on the 693!?!?

I read a lot of threads on this when we were preparing our AOS package, and there are a ton of threads and opinions out there. There are lots of people that have submitted the vaccination sheet from Dr. Seiden and gotten approved with it without any problem. There were a few people who had gotten RFEs, but that looked rare and there could be a million underlying unknown reasons.

We wavered on whether to submit the vaccination supplement or to do the 693 supplement, because we filed our K-1 a year ago, we're tired, and the thought of an RFE that could delay us made us nauseous (I know we're all in the same boat). When we were deciding which to do, I called the civil surgeon's office to ask cost, how long it would take, what they needed, and if the USC could drop off/pick up the Canadian spouse's info. With those details, we made the decision to send in our packet with the vaccination supplement we got from Dr. Seiden, because we knew that if we had an RFE on this issue, we could get it taken care of in less than a day for $40 with the papers from Dr. Seiden. Plus, I (the USC) could drop off the papers in the morning and pick them up on my lunch hour - the CS is less than 5 minutes from my office - to get the RFE sent out that same day.

That was our decision, until our marriage certificate got delayed due to a vacation of the marriage cert. clerk (can't get a certified copy without the original certified certificate). That delay of only a few days made us think that having the vaccinations transcribed by the civil surgeon wasn't expensive, didn't require us to travel, and wouldn't risk any delay. So we changed our minds, and had the vaccination supplement transcribed onto the 693. I dropped off the papers in the morning, picked the sealed envelope up at lunch, changed our cover letter that night, and then we still waited another two days for our marriage certificate.

That's what we did and the explanation as to why - that was the right choice for us for our situation. It may not be the right choice for anyone else.
