Please help with I-693 form

I am about to submit my form right now.I dont really know what to do as regard to form I-693.Most people that submitted form here on adjustment of status dont normally add that form.But I read in the instruction that I must submit the form along with it.Do I have I have to meet a civil surgeon and do my test and sign on before submiting the form.Please I want someone to help me asap

hi there..u seem to need lil help there...yes, u should see a local civil surgeon in ur place to transcribe ur vaccination records to I-693 form...if u need more shots here in the US, u can either have those shots in any local health clinic in ur place or if money isnt an issue to u, u can get the shots in ur civil surgeon's clinic..they also administer it there but price of shots may be costly. I can tell u to keep on looking and call the CS on the fon and inquire about their fees and there u can decide which one offers the best service at a less cost..Some peeps here say some CS try to rip off their applicants by charging way to high..another thing, if ur medical in ur country (before u came to US) is less than a year, ur not supposed to have a complete medical here in the US..just fill out parts 1,2 and 5 of 693 and make sure u sign the form in the presence of ur CS..he/she needs to sign there as well...after all these, u will be given a sealed I-693(for submission with ur AOS packet) and ur personal copy..make sure u wont open the sealed copy..it wont hurt u but u can also include a photocopy of ur vaccination record(in ur country where u had ur first medical for K-1)in ur AOS packet...

Just so u know, some people here get an RFE(request for evidence)for I-693 because they failed to submit it..It will only delay the process then..

What type of visa are you adjusting status from?

If you are adjusting from a K visa and have had a medical exam within the past year then you are not required to have another medical exam but you are still required to have the vaccination section of the I-693 completed and signed by a USCIS approved Civil Surgeon.

I believe if you are adjusting from a student or visitor visa you are required to have a complete medical exam performed by the Civil Surgeon.

I understand that unless you have already submitted a medical for the visa you entered on, you have to file the I-693 to AOS.

Shop around for your civil surgeon, though, truth be told, there were not that many in my area. It cost us a total of $325. That included the physical, the cost of the test for syphalis, and the vaccinations which our doctor recommended we get at our local health unit. It only cost $10 there for the vaccinations.

AOS Timeline

4/14/10 - Packet received at Chicago Lockbox at 9:22 AM (Day 1)

4/24/10 - Received hardcopy NOAs (Day 10)

5/14/10 - Biometrics taken. (Day 31)

5/29/10 - Interview letter received 6/30 at 10:30 (Day 46)

6/30/10 - Interview: 10:30 (Day 77) APPROVED!!!

6/30/10 - EAD received in the mail

7/19/10 - GC in hand! (Day 96) .

I just called a place now and they said $148

That sounds like a bargain to me. Did you select a CS from the USCIS approved list?

We paid $50 just for the vaccination translation and my wife didn't even see the doctor. The girl in the office completed the papers and the Doc just signed it. Geez, $50 bucks for a 2 second signature that you can't even read, I'm in the wrong business :hehe:

Yeah i would suggest, do I-693 and meet a Civil Surgeon. I think it would be better to do it now to avoid RFE related to this subject.

wow thats a lot! for my i-693 i paid $80 that included physical, tb test and blood work, I went to my county health department and got my shots for free. Go here and call all civil surgeons in your area and ask for price https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV
