I will be going tomorrow to get the form 693 supplement completed (transcribing the vaccination records) by a Civil Surgeon.

My question:

Does she use her new married name (now that she is married to me) or her previous name when she was my fiancee - the original medical was done overseas in her previous name?

She came here on a K-1 (fiancee visa).

Thanks.

I think immigration is smart enough to understand that some things will be in her old name and some in new name, but what is important is to adjust the status in the new name. When we did ours, we just gave them the vaccination paper from the exam and her passport and then they filled it out in her old name, they used their own form, we didn't even have to bring our form.

We didn't have any problem getting in the immigration building either, one building did ask to see the marriage certificate since they had her new name on the appointment letter, and the next building didn't even ask to see it, they were smart enough to realize that people change their name when they get married, but I have read that some people had problems getting in the building, must have been stupid people at that office.
