Hello everyone, pls I have some questions about AOS, we received a letter today from USCIS saying they need my W2s, medical record and vaccination report. My husband did his medical before coming here on a K1 vis, we sent the vaccination report form to USCIS when we submitted the AOS application but I guess that is not enough. I lost my job in January of this year but I sent them my 1040 paper for 2008 and 2009,

1.My question is do I just send them the W2s or do you guys think I need a co-sponsor?? It didn’t say I need a co-sponsor in the letter , they just said I need to send my W2s that I submitted to IRS, I haven’t work since I got laid off cuz I was pregnant at the time and I just gave birth a month ago.

2. I called a local civil surgeon, the doc said they don’t do vaccinations but they do medical exam for $190, so do I look for another doc that do vaccination or do I submit the vaccination repot form that my husband brought from doing his medical in Nigeria.

Please I do not want them to denial our application and I want to make sure I have everything before I send it again, any help will be appreciated. thanks

If she has the vaccination report, Civil Surgeon only need to check if she have the shots needed then transfer the records on form I-693. No need for another medical exam.

July 21, 2010 - I-485 GC approved, for card production

July 26, 2010 - Welcome letter received

July 28, 2010 - EAD card received

July 31, 2010 - Greencard received

Removal of Conditions

May 7, 2012 - Date Filed I-751

May 9, 2012 - Date of NOA!

yes we have the vaccination report so you saying we do not need another medical exam or vaccination, we just have to have the civil surgeon tranfer the reocrds on the form I-693. In the form I-693, they said the civil surgeon must have the result sealed and must not be opened?? what about that, are they talking about people who need a new medical exam or evybody?? thanks

If she came here with a K1 visa and have her medical examination done prior coming to US (if she had the medical within 1 year), there's no need for a new medical exam. If you are applying for adjustment of status: Submit Form I-693 together with your Form I-485, Application to Register for Permanent Residence or Adjust Status, as detailed in the instructions to Form I-485 (please read I-485 and I-693 instructions). She only need to filled-up part 1 of the I-693 and need to sign it infront of the Civil Surgeon. The Civil Surgeon will transfer her vaccination records on the form I-693, sealed it in an envelope with a note "DO NOT OPEN FOR USCIS USE ONLY". You can ask for copies on what's inside the envelope.

thanks alot for your help, i know what to do now but the Civil surgeons i contacted all told us we need to complete a new medical exam for around $190, i read about Dr Anold in texas but they said she has retired, can anyone let me know if there is another doc like Dr Anold willing to transfer the vaccination report to the form I693.Pls let me know and am still waiting for question Num 1 to be answered by anyone willing to help me. thanks so much
