Do I need I-693 or 693 supplement?

I am totally confused and keep reading/hearing conflicting information about which document I need. My wife came to the US from South Korea on a K1 visa. She had her medical exam done in Korea and got her vaccinations done last week. I spoke with someone from the USCIS today and they said she needs to complete part 1 of the I-693, while the civil surgeon should complete parts 2 and 5. However, I've read on this site that she only needs an I-693 supplement since her vaccines were done here. I can't even find a place to download the I-693 supplement. Can anyone clarify what exactly my wife needs to have the civil surgeon fill out? I've called around several different places and nobody seems to know.

I spoke with someone from the USCIS today and they said she needs to complete part 1 of the I-693, while the civil surgeon should complete parts 2 and 5.

That is it on the button. My wife went last week and that is exactly it. What they didn't tell you is that the civil surgeon only needs to complete the vaccination section of part two (I belive it is page 5).

You complete Part 1 (but do not sign it yet - the CS will have you sign it upon completion)

The CS will transcribe the vaccinations

The CS will complete and sign Part 5.

They will seal it in an envelope and give it to you. Make sure you get a copy also.

We paid $50 and I have seen alot who were only charged around $35. All depends on where you live and how many you have to call to find one that knows what they are doing.
