Earnings and Leave Statements?

I had a quick question, we are compiling all the necessary documents and I had a question on this one.

What would be Earnings and Leave Statements?

I printed out my 401K statement for the past year, as well as my Sick Leave statement, Vacation time statement, and my Compensatory Time statement.

Would that be it?

Add these to the list, from what I have read if you are above the 125% poverty line these should be sufficent:

1) pay check stubs for the last 6 months

2) letter of employment from employer

3) 2008 W-2 and tax returns

4) 2009 W-2 and tax returns

09-21-2009 - Met for the first time

10-31-2009 - Visited for 5 days

12-30-2009 - visited for 11 days (informal engagement)

1-13-2010 - Mailed I129F Petition

1-15-2010 - Recieved NOA1

1-21-2010 - Touched

3-09-2010 - visiting for 6 days

3-12-2010 - Formal Engagement in Ochee

4-20-2010 - Recieved NOA2 (no previous touches)

4-22-2010 - NVC Recieved my Petition, Assigned New Case#

4-23-2010 - Case leaving NVC (have DHL tracking# so I can follow case to Kingston)

4-23-2010 - Recieved NOA2 (hard copy) in the mail

4-26-2010 - Kingston Embassy Recieved Petition (signed by Mr. Morgan)

4-25-2010 - visiting for 5 days

4-30-2010 - Kingston Embassy mailed out Packet# 3

5-3-2010 - E-Mailed Kingston Embassy DS-230

5-4-2010 - Embassy e-mailed interview date

5-26-2010 - Medical

6-6-2010 - Flying down for interview

6-8-2010 - INTERVIEW...APPROVED!!!!

6-23-2010 - VISA RECIEVED!!!

6-27-2010 - Flying down to Jamaica

06-30-2010 - POE (JFK)

09-25-2010 - WEDDING!!!

Add these to the list, from what I have read if you are above the 125% poverty line these should be sufficent:

well I was doing a research grant for all of 2008 with the university I attended in 2008, so I was WELL below the poverty line

I already got the letter from my employer, and I can get the pay check stubs no problem

The only one that will give me trouble is the 2008 W-2, I have no idea where that is.

What I have sent her is the following:

* 2009 Federal Tax Return (1040)

* Copy of Bank Account statement from April 2010

* 2009 W-2 form

* Letter from employer stating Yearly salary, title, fulltime status, and date of hire

Im working on the I-134 right now. Im having trouble finding out these questions on #7

I have other personal property, the reasonable value of which is:

would that be cars, electronics et cetera?

With a cash surrender value of:

I figure I could just contact my HR department about this one and see what my benefits state

Would a 401K be considered a stock or a bond?

If your income is currently at 125% of the poverty line, you do not need to list the assets. They are not interested in that unless you are using assets to cover a shortfall in income to reach the 125% of the poverty level. If you are at or above the 125% line, you do not need bank statements or 401K listings or anything of that sort.

You can call the IRS at 1-800-829-1040 and go through the automated system to request a tax transcript for 2008 (and even 2009 if you want) which makes the W-2 unnecessary. I believe if you are in a hurry to get the form you can have it faxed to you within the next business day.

I just had three years mailed out to me and it took 10 days to receive them.
