Hello everyone,

I've just gotten my vaccination medical worksheet with the results. I have a question and hope someone can help me with an answer. on the bottom of the form it is "X"d on the top box that says "Applicant may be eligible for blanket waivers as indicated above"..and then another X below it which says "applicant may be eligible for blanket waivers because vaccinations not medically appropriate (as indicated above)"....The box that says "vaccine history complete for each vaccine, all requirements met", is not check marked. Is this going to be a problem? The doctor did not mention anything to me about my immunization list, or stated I needed anything.

Hopefully I'm worrying for nothing, please feel free to let me know if this is normal or something to worry about...

Yes that's normal, it just means that some of the vaccinations were not age appropriate for you.

So the box next to "vaccine history complete for each vaccine, all requirements met" Does not need to be marked then?

they can't mark both boxes. it's in the technical instructions for panel physicians. they are very specific on which boxes can be check and when or not.

nothing to worry about. can't remember the link now but it was a pdf i found on uscis website.
