I believe blank or a dash is the best option. I used blank for my fiancee's middle name and that worked fine with USCIS.

DoS assumes NONE and NA (but not N/A) can both be legitimate names and if appearing on the new DS-156 which is processed electronically, will be removed. You can see evidence of this on the 3rd page of the DS-156 output after you have submitted it and received the final copy with bar code.

My fiance also has no middle name, so we just put a dash in all locations. USCIS and US Consulate have had no problems yet. Just make sure any legal documentation (birth certificate, etc.) also reflects no middle name.

Yeah - the "-----" (line) is now my favorite method - indicates you answered the question. Used to be in favor of "No Middle Name" until one of Karin's NOA's was issued with that as the middle name (no kidding).

You and your fiancee MUST have a middle name. If you do not currently have one, you MUST first change your name legally and submit the court documents with your petition. Middle names are required for ALL applicants as that is how they determine if you are a terrorist or nor...choose carefully.
