Timeline

Okay I am almost down to the end, but I have all of these supplements needed. How do you do them?

Example: For every box that was too small and needs more room for address etc. Do you do a new supplement for each one?

I have a weird long name and on my birth certificate I have my full long name, but on my social security card it has only my shortened name and middle name and last name (missing my full regular first name, and my mom's madden name which is on my birth certificate). My whole life I have filled out anything dealing with money (social security card) with just the shortened name, anything with my legal name-my long birth certificate name. Can I just do a supplement showing a copy of my birth certificate and social security card to show this and sign it?

Help, I am really freaking out about everything, wanting it all to be perfect. I so much want my wonderful Fiance to be here and I don't want to screw stuff up that will delay it.

Create a page titled supplement to form XXXXXX and then page 1 of 1 ( or however many supplement pages you make. )

Also include any standard form header data common to the form

Then for each section you are using

Supplemental data for item YYYY

(data for that section in the form of the section )

Supplemental date for item WWWW

(data for that section )

At the end of each page sign and date it .

For each entry that has extra data enter SEE SUPPLEMENT in the regular part of the form

This will not be over quickly. You will not enjoy this.
