Hey all!

I've been preparing for this phase of the process and when I looked at the flowchart I noticed that it is necessary to fill out

an I-693 form to prove a medical examination was performed. The link, however, only leads to the USCIS page and a search of their website does not provide the form. Does anyone know anything about this form and where I can obtain it?

I cant seem to find it and I would like to fill this out as soon as possible.

To navigate there you go to www.USCIS.gov and then click on the big green word at the top of the page that says "forms". That takes you to the page I linked. The form is called I-693. They forms are MOSTLY in numerical order (it's a little weird) but pressing Ctrl+f and then typing "I-693" will yield the form.

You shouldn't need the I-693 as the USCIS civil surgeon should have them. Get a copy before they seal the envelope tho.

They do yes, mine had a completely incorrect and out-of-date form which I found out AFTER they completely F'ed it up but luckily I noticed before I sent it, so I went back with my OWN version, the correct and up-to-date one from the website and had them complete it.

Also, I wanted to type my details on Page 1 and assumed they would just add my page 1 to the rest of the pages.. which they did, but the other pages were completely wrong.. ugh. Complete mess (esp as the I-693 says only certain versions are accepted). Better safe than sorry!
