Hi guys - chiming in on the confusion. I am done my AOS stuff except for the vaccination situation. All I got at Medisys in Toronto was a folded yellow card that says "Immunization Record" on it, with my dates and the administiring nurse's signature.. I am guessing that's not the DS 3025 we are talking about from the looks of it.

So am I beating a dead horse here assuming I need to call Medisys????? Grrr!!!!! Would they have stuck it into the DO NOT OPEN ON PAIN OF DEATH envelope the medical exam was in? I only had the damn thing done in March 2006 so I know I don't need them again. Oh, I still have chest X Ray - they didn't take at at the interview and I kept it.

Yes, the DS-3025 would've been in the mystery envelope, they also should've given you a copy.

The yellow booklet may be enough to take to a US Civil Surgeon to get your I-693A if that's what you're worried about, you might try to call Medisys to see if you can also get a copy of the DS-3025.

I will just add my personal experience.

When I went for my K1 interview in Montreal I did not have the chest x-ray or tb skin test as I was pregnant at the time of my medical. When approving my visa the officer informed me that I should get a tb skin test done once I get settled in the USA before my interview.

I just went and got the test last week and my interview is tomorrow...I will see if they even glance at the document.

My husband had to have a TB test. They use one needle and you have to go back in 48 hours to get it read.

Did your husband have a chest x-ray done as well?

I was just going to say, if you had the xray done you don't need the skin test unless something showed up on your xray. Check your xray, there should be something noted on it, or call the Dr. you went to your K1 medical for, they should be able to tell you if anything showed up. You can also do an advanced search on here for +skin +test, that will bring up all topics about it to see if you fall into certain employment categories that might need to get the skin test (nurse, etc.)

The only records I brought down with me are for the 'female parts' doctor, the rest I'll have faxed as needed (including dentist).

My chest x-ray was okay, so I guess I won't be needing the TB test after all! *relieved*

I went to the Dr today with my yellow card - no problem. I had a long discussion with the receptionist before I went, she had no idea what I was talking about and asked me to bring the I - 693a if I had it (so I printed it off the USCIS website) and when I got there, they had it already. I was there for 5 minutes, explained to the nurse what had to be done, she got the dr, he took my paper away, wrote it out and gave me a copy and a sealed one. All good. next day appointment and less than 2 miles from my house.

Timeline

The skin test is usually administered first. If the site reading is approximately 10 mm or larger (PPD positive) after the 48 hour wait, that is when chest xray is prescribed, just to ensure lung is clear. But once patient has a positive reading, the skin test should never ever be again administered, as reaction can be severe.
