do i need a cover letter to return my checklist ?if yes.can anybody tell me how to do it?thanks9

It isn't mandatory but it is suggested. You can go into the wiki section or do a search for "checklist response cover letter".

When I did a search in the wiki after posting this, there wasn't a template unfortunately. However, this is what is suggested to include in the cover letter:

- case #

- name and dob of the beneficiary

- name and dob of the petitioner

- explain that this is a checklist response

- list everything that you've included in the package

This should suffice. Also be sure to include the checklist letter that was sent to you and write your case # on each of the pages you submit.
