WE ARE FILLING OUT THE G-325A FOR YOKO'S AOS PROCESS AND WE HAVE A SIMPLE QUESTION. IS SHE FILLING OUT THE G-325A WITH HER MAIDEN NAME OR MARRIED NAME? WE JUST WANT TO FILL THIS OUT RIGHT THE FIRST TIME. THANKS

If she has taked your family name as her married name then that is what she should put.

She should also then put her maiden name in the field below where it asks for "All other names used".
