In what circumstance would my wife need to retake her medical exam, if we are past the expiration date which is 03/31/2011 or a year after the exam date which is 05/11/2011

Your wife doesn't have to re-take the exams.. the DS form will not be valid if is over a year.. The reason of the I-693 is to ensure all your vaccinations are up today and the CS will signed and testified the facts.

The one year rule applies when you want to use the DS2035 form rather than sending a sealed medical form [i693].. typically, RFEs are issued when no I693 are presented.. is recommended to visit a CS to avoid this possible step back
